What is the Patient Portal?
The Patient Portal makes your health information more convenient to access. It is a secure, confidential, and easy-to-use website that allows patients and their families 24-hour access to their medical records. It uses the latest encryption technology to deliver secure communication between patients and providers.
How Can I Gain Secure Access to my Account?
You can gain access to your patient portal account in 2 ways. If you need BPMC staff member to help you to complete the enrollment process, the front desk staff will assist you to set up your account by providing you with a username and temporary password. At BPMC, you can also visit our website, download the Healow app and the staff member will walk you through the entire enrollment process.
If you decide to complete the enrollment process at home or on your smartphone, you will also need to go to BPMC website (www.brooklynplaza.org) and you can either enter the user name and temporary password you received from the front desk staff or you can create your own username and password. You will follow the system's instructions to open your private account.
Do you offer support for clients in need?
No one is denied care because of an inability to pay. Brooklyn Plaza Medical Center offers a sliding fee discount program for both insured and uninsured patients based on an individual’s income and family size. Staff is also available to assist with applications for the child health insurance plan (CHIP), free or low cost insurance for children fewer than 19 and family Health Plus for adults 19-64 years of age.
What are your policies regarding patient privacy?
Federal HIPAA Privacy Rules establishes national standards to protect individuals’ medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. HIPAA also gives patients’ rights over their health information, including rights to examine and obtain a copy of their health records, and to request corrections.

Brooklyn Plaza Medical Center, Inc. is recognized by The National Committee for Quality Assurance (NCQA) for our Patient Centered Medical Home model. We have achieved Level 3 which is the highest designation!

To help us provide the best care, please bring the following items with you:
- Identification: A valid photo ID (e.g., driver's license, state ID, or passport).
- Insurance Information: Your insurance card(s) or proof of coverage (if applicable). If you are uninsured, bring proof of income and residency if you're applying for our sliding fee scale.
- Medications: A list of all current medications, including over-the-counter drugs, vitamins, and supplements. Bring the actual medication bottles if possible.
- Medical Records: Any recent test results, imaging reports, or referrals from other healthcare providers. If you're transferring care to us, bring copies of your medical history.
- Questions or Concerns: Write down any questions, symptoms, or concerns you want to discuss with your provider to ensure nothing is overlooked.

Are Same-Day & Walk-In Appointments Possible?
We understand that health concerns can't always wait. Same day appointments are available for urgent medical needs. Call 718-596-9800 to check availability. Walk-ins are welcome, but we recommend calling ahead to minimize wait times.
What If I Need Care After Hours?
If you have a medical emergency after regular hours, please call 911 or go to the nearest emergency room. For non-emergency concerns, our call center is available 24/7 at 718-596-9800 to connect you with a BPMC doctor or our answering service.
Can I Schedule a Virtual Appointment?
Without leaving your home, you can communicate face-to-face in real time with a BPMC provider through your smartphone or tablet device. You will be connected within minutes for your scheduled, telehealth video visit session, in which the provider can provide a diagnosis, next steps and prescribe medication, if necessary. Our providers can consult with you on various areas of specialties. Virtual visits are covered by Medicare and most major insurances. Here's how to schedule an appointment:
- Call Us: Schedule your telemedicine appointment by calling 718-596-9800. Let the scheduler know you'd prefer a virtual visit.
- Use the Patient Portal: Log in to our Patient Portal to book your telemedicine appointment online.
- Request During an In-Person Visit: Ask your provider if telemedicine is an option for your next appointment.
I am New to BPMC. Will I Need to Complete any forms?
Yes, you will be asked to review and complete various forms when you arrive at the practice such as:
- Notice of Privacy Acknowledgement/Consent Form
- BMPC Demographic Form
- Statement of Financial Responsibility
- Consent for Medical Treatment
All of these forms can be reviewed and completed prior to your appointment by visiting our patient forms.

What forms of payments do you accept?
For your convenience, our practices accept cash, personal checks, money orders, and credit cards including Visa, MasterCard, Discover, and American Express.
We do not accept traveler's checks. We will charge your credit card only for the amount you authorize.
Payment plans also may be arranged through our onsite billing coordinator.
