Telehealth Services
There is no need to place your care on hold during the COVID 19 pandemic. Brooklyn Plaza Medical Center is currently offering virtual services. Without leaving your home, you can communicate face-to-face in real time with a BPMC provider through your smartphone or tablet device. You will be connected within minutes for your scheduled, telehealth video visit session, in which the provider can provide a diagnosis, next steps and prescribe medication, if necessary. Our providers can consult with you on various areas of specialties.

Substance Abuse Treatment Services
BPMC offers evidence-based substance abuse treatment services for those individuals struggling with addiction. The substance abuse team at BPMC provides professional and specialized treatment for people with additions. BPMC has developed an integrated team consisting of substance abuse counselors, HIV specialists, doctors, social workers, and psychiatric nurse practitioners. Therefore, BPMC's clients who are dually diagnosed with mental health and substance use disorders are able to receive an array of specialized services. Our staff is highly trained in treating at-risk groups, adolescents, adults, seniors, and the LGBTQI population, as well as families who are experiencing homelessness and the formerly incarcerated.
GROUP THERAPY
We offer group therapy, such as early recovery group, relapse prevention group, wellness group, anger management group, and re-entry group. BPMC offers a safe and protective group environment where individuals can share their thoughts and concerns. The group sessions are conducted by an addiction specialist, using motivational interviewing approaches and cognitive behavioral therapy. Individuals are encouraged to share their experiences and provided with positive feedback from their peers. Group sessions are offered during the week and they are held in the morning and Tuesday evenings.
ONE-ON-ONE INDIVIDUAL SESSIONS
One-on-one sessions are conducted between the patient and his/her assigned addiction counselor. The patient will meet weekly or biweekly, with the assigned addiction counselor, depending on his/her individualized treatment plan. The patients are encouraged to fully participate in these one-on-one sessions to develop coping skills in order to help them recognize and combat triggers that can lead to a potential relapse. These sessions are offered in a safe and pleasant environment where confidentiality is respected.
NICOTINE CESSATION
Tobacco use can lead to tobacco dependence and serious health problems. Tobacco/nicotine dependence often requires repeated treatment. BPMC addiction counselors conduct smoking cessation sessions to help those individuals struggling with tobacco abuse/dependence. The addiction counselor uses a harm reduction approach and nicotine replacement therapy (gum, patches, or Chantix) to assist the patient.
TOXICOLOGY TESTING
The addiction counselor will monitor the patients' toxicology reports on a regular basis for the purpose of monitoring treatment progress. Determining a patient's next level of care or potential completion will be clinically determined based on the patient's toxicology reports.
HIGHER LEVEL OF CARE
BPMC will provide a higher level of care to those individuals with chronic relapses. A clinical team approach will be used to determine if a patient might benefit from a higher level of care, such as detox, 28-day rehab, or long-term inpatient care.
MEDICATION ASSISTED TREATMENT
Medication-assisted treatment (MAT) is the use of medications, in combination with counseling and behavioral therapies to provide a “whole-patient” approach to the treatment of substance use disorders, such as opioids and alcohol. BPMC offers the following: Medication Assisted Treatment, Suboxone, Naltrexone, Vivitrol, and Antabuse. The patient will be medically assessed to determine if his or her opioid or alcohol use disorder warrants such treatment. The medical provider and addiction counselor will monitor the patient's treatment progress on a daily, weekly, or biweekly basis.
NALOXONE AND FENTANYL TEST STRIPS TRAININGS
BPMC is a Licensed Opioid Overdose Prevention Program. To reduce the number of overdoses in the communities, BPMC provides opioid overdose prevention training and naloxone nasal spray. BPMC recommends that individuals, including families, friends, and those who are personally at risk for an opioid overdose be trained and carry a Naloxone nasal spray. BPMC also provides Fentanyl Test Strips and training on how to use Fentanyl Test Strips. There are different-like drugs which can cause a person to overdose quickly. It is hard to tell the difference between them all by just taste, smell, color, or feel. Fentanyl test strips are easy to use to determine if a user's drugs have fentanyl in it. When the person gets the results, then a decision can be made as what to do next
TELEPHONIC/TELEMED/FACE-TO-FACE
BPMC offers substance abuse services through telephonic, telemed, or face-to-face visits.
Brooklyn Plaza Medical Center programs are operating under the regulations and standards of several government agencies. We are:
1. Licensed by the NYS Department of Health to provide primary care.
2. Registered with the Drug Enforcement Agency to store and dispensed controlled substances.
3. Licensed by NYS Department of Health as an Opioid Overdose Prevention Program.
