Brooklyn Plaza Medical Center, Inc. (BPMC) is dedicated to improving the physical, spiritual, psycho-social and psychological well being of residents of North Central Brooklyn by providing the highest quality preventive comprehensive family health services with particular concern for lower-socio-economic groups.
Outreach Brooklyn
In our effort to continue to promote a healthier Brooklyn, twice a month, the medical staff of Brooklyn Plaza Medical Center, Inc. brings dependable medical services to Community Centers, Community Based Organizations and Colleges throughout Brooklyn. The screenings will range from simple exams and portable screening tools and questionnaires for diabetes, obesity, high cholesterol, hypertension, alcohol abuse, and depression. By bringing these services to the residents, they will be able to access care that was unavailable due to lack of options.
The Rising Heights Program
Brooklyn Plaza Medical Center, Inc.’s HIV/AIDS Department, the Rising Heights Program (RHP), has a successful 16-year history of providing comprehensive, high quality HIV/AIDS healthcare services within a Community-Based Health Center. The local community bares a significant burden of the HIV/AIDS epidemic and engaging and maintaining HIV/AIDS patients in care is a significant challenge. North and Central Brooklyn neighborhoods include areas of endemic poverty and homelessness. Many residents face language and cultural barriers, particularly those with special needs such as untreated co-morbid conditions including mental health and substance abuse problems. Linkage to care, maintenance-in-care, and treatment adherence education services are labor and resource intensive, yet critical to optimizing patient health outcomes.
The RHP personnel apply a quality assurance plan through which comprehensive services are integrated throughout the network of providers. The plan includes assessment and improvement actions to advance program performance, maintain a high standard of service delivery and maximize clinical outcomes. Additionally, RHP personnel conduct prevention education outreach to provide the community with the knowledge and skills needed to prevent HIV transmission. Outreach and educational initiatives connect health professionals with hard-to-reach populations. Participation in regional consortiums and collaborations link the RHP providers with community based organizations and strengthens the community’s capacity for HIV prevention and care.
The RHP provides the highest quality of care for those living with HIV/AIDS in a warm, friendly, and professional environment. Patients receive individualized care from an experienced physician who has practiced HIV medicine for decades. Case managers and all clinical staff are compassionate, committed and qualified in ensuring the patients’ health needs are met to their satisfaction.
Barbershop Initiative
Partnering with neighboring barbershops, Brooklyn Plaza Medical Center, Inc. has put together "The Barbershop Initiative" geared to making sure men in our community are aware of their health status. Screenings in hypertension, diabetes and asthma as well as health information is given by medical professionals from BPMC. Promoting a healthier Brooklyn has been the goal of Brooklyn Plaza for the past thirty-seven years. Reaching men through the Barbershop Initiative has proven to be the best way to educate and drive home the importance of taking preventative measures to insure their health and well-being.
Medication-Assisted Therapy (MAT) Prescription Drug and Opioid Addiction
MAT is an expanded service that supplements our Behavioral Health program. This program focuses on reaching those in our community that is in an increasing need for opioid use treatment. The program links patients that screen positive for opioid use to a MAT specialist for ongoing support and treatment. MATs overall goal is to expand patient access to and retention inappropriate substance use disorder treatment, behavioral health, and HIV/AIDS and viral hepatitis services. It focuses on providing these services through two evidence-based practices: 1) Screening, Brief Intervention, Referral, and Treatment (SBIRT) and 2)a Twelve-Step Facilitation Outpatient Program, specifically to individuals with substance use conditions. For clients in need of long-term or intensive mental health services or substance use treatment, referral to external services organizations to ensure continuity of care is offered.
Major Types of Diabetes
Type 1 Diabetes
Results from the body's failure to produce insulin, the hormone that "unlocks" the cells of the body, allowing glucose to enter and fuel them. It is estimated that 5-10% of Americans who are diagnosed with diabetes have type 1 diabetes.
Gestational Diabetes
Immediately after pregnancy, 5% to 10% of women with gestational diabetes are found to have diabetes, usually, type 2.
Type 2 Diabetes
Results from insulin resistance (a condition in which the body fails to properly use insulin), combined with relative insulin deficiency. Most Americans who are diagnosed with diabetes have type 2 diabetes.
Pre-Diabetes
Pre-diabetes is a condition that occurs when a person's blood glucose levels are higher than normal but not high enough for a diagnosis of type 2 diabetes. There are 57 million Americans who have pre-diabetes, in addition to the 23.6 million with diabetes.
Diabetes Collaborative
Diabetes is a disease in which the body does not produce or properly use insulin. Insulin is a hormone that is needed to convert sugar, starches and other food into energy needed for daily life. The cause of diabetes continues to be a mystery, although both genetics and environmental factors such as obesity and lack of exercise appear to play roles.
There are 23.6 million children and adults in the United States, or 7.8% of the population, who have diabetes. While an estimated 17.9 million have been diagnosed with diabetes, unfortunately, 5.7 million people (or nearly one quarter) are unaware that they have the disease.
In order to determine whether or not a patient has pre-diabetes or diabetes, health care providers conduct a Fasting Plasma Glucose Test (FPG) or an Oral Glucose Tolerance Test (OGTT). Either test can be used to diagnose pre-diabetes or diabetes. The American Diabetes Association recommends the FPG because it is easier, faster, and less expensive to perform.
With the FPG test, a fasting blood glucose level between 100 and 125 mg/dl signals pre-diabetes. A person with a fasting blood glucose level of 126 mg/dl or higher has diabetes.
In the OGTT test, a person's blood glucose level is measured after a fast and two hours after drinking a glucose-rich beverage. If the two-hour blood glucose level is between 140 and 199 mg/dl, the person tested has pre-diabetes. If the two-hour blood glucose level is at 200 mg/dl or higher, the person tested has diabetes.

For the first time ever, many New Yorkers will have help paying for a health plan. Through an Advanced Premium Tax Credit, health insurance premiums will be more affordable to a wider range of people; from an individual earning $45,960 to a family of four making $94,200.
Support for the “extras”
It’s not just the monthly insurance bill that's a burden. It’s the extras, like “co-pays” (the part of the medical bill that’s charged to you not your insurance company) and “deductibles” (the amount you must pay before your insurance kicks in). Through Cost Sharing Reductions, the out of pocket costs are reduced and if your income falls below a certain point; NY State of Health will help you pay for these “extra costs”. Cost Sharing Reductions are limited to certain health plans.
Everyone in the pool
Insurance rates are based on the number of people in a group, or “pool.” The larger the pool, the lower the monthly cost of health insurance for everyone. The pool in NY State of Health is going to be very large.
Healthy or sick, the cost is the same
No one will be denied coverage or charged more just because they are already sick.

340B Prescription Drug Discount Program
Brooklyn Plaza Medical Center has partnered with local pharmacy operators to simplify our patients’ access to prescription medications. In addition, patients who are eligible for our Sliding Fee Program can receive discounts made possible by the U.S. Department of Health and Human Services, Office Pharmacy Affairs, by utilizing pharmacy partner locations.
The revenue generated from the 340B Pharmacy Program enables us to expand access to care and medicines for patients with insufficient financial resources.
