Greetings and Welcome to BPMC, 



While we continue to face unprecedented times, Brooklyn Plaza Medical Center would like to assure you that we are following precautions to keep everyone safe. We are practicing social distancing recommendations, strict hand washing guidelines and cleaning protocols. Our providers are available to see you for essential in-office appointments or we can consult with you through video visits. 



Currently, our 650 Fulton Street location is open Monday –Friday from 8am-5pm and Saturday 10-3pm and our WIFHC location is slowly reopening.





Here’s what you can expect from BPMC and WIFHC locations during this time, 

  1. Patients and staff are required to wear masks.
  2. Upon arrival, patients will be asked to complete specific COVID 19 screening questions and their temperature will be checked.
  3. We have reduced the capacity of our elevator and waiting rooms to help with social distancing. 
  4. We kindly ask that only patients with a scheduled appointment enter the Center. We recommend that companions remain in his/her vehicle to limit the number of people in the waiting room. 
  5. If you have a caregiver who needs to attend the visit with you, he or she will also need to wear a mask. Please inform staff if this is the case.



If you are not clear as to whether you need an in-office visit or video visit from your home, please call your provider. If our provider determines that a video visit is best for you, we will provide instructions for a video visit.

Brooklyn Plaza Medical Center, Inc. offers a full range of health care and support services. Staff is also available to assist with applications for the child health insurance plan (CHIP), free or low cost insurance for children fewer than 19 and family Health Plus for adults 19-64 years of age.

Sliding Fee:  No one is denied care because of an inability to pay. Brooklyn Plaza Medical Center offers a sliding fee discount program for both insured and uninsured patients based on an individual’s income and family size.

FTCA: Brooklyn Plaza Medical Center is a Federally Qualified Health Center, receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals

HIPAA Privacy Rule
The HIPAA Privacy Rule establishes national standards to protect individuals’ medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. The Rule also gives patients’ rights over their health information, including rights to examine and obtain a copy of their health records, and to request corrections. The Privacy Rule is located at 45 CFR Part 160 and Subparts A and E of Part 164.
