    The MAT program links patients that screen positive for opioid use to a program specialist for ongoing support and treatment. 

  • 340B Prescription Drug Discount Program

    We partner with local pharmacy operators to simplify our patients’ access to discounted and free prescription medications made possible by the U.S. Department of Health and Human Services and the Office of Pharmacy Affairs.

The Importance of Cultural Sensitivity Cultural sensitivity refers to the awareness and understanding of the cultural differences that exist among patients. These differences can encompass a wide range of factors, including ethnicity, language, religion, and socioeconomic status. When healthcare providers approach patients with cultural sensitivity, they demonstrate respect for their patients' backgrounds and perspectives, which can significantly enhance the patient-provider relationship. Brooklyn Plaza Medical Center, Inc. offers a full range of health care and support services. Staff is also available to assist with applications for the child health insurance plan (CHIP), free or low cost insurance for children fewer than 19 and family Health Plus for adults 19-64 years of age.
Brooklyn Plaza Medical Center, Inc.’s Leadership is devoted to promoting the well-being of our Human Capital through a comprehensive health and stress reduction perspective involving the physical, emotional and spiritual domains. Sliding Fee:  No one is denied care because of an inability to pay. Brooklyn Plaza Medical Center offers a sliding fee discount program for both insured and uninsured patients based on an individual’s income and family size.

FTCA: Brooklyn Plaza Medical Center is a Federally Qualified Health Center, receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals

HIPAA Privacy Rule
The HIPAA Privacy Rule establishes national standards to protect individuals’ medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically. The Rule also gives patients’ rights over their health information, including rights to examine and obtain a copy of their health records, and to request corrections. The Privacy Rule is located at 45 CFR Part 160 and Subparts A and E of Part 164
