As a patient, you have the right to access and obtain your medical records whenever you need them. This includes visit summaries, test results, immunization records, and more. To support you in exercising this right, we have established a clear and straightforward process for requesting your medical records, whether in person, by email, fax, or mail. Your Rights Under HIPAA

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) ensures that you have the right to:
- Access your medical records promptly.
- Obtain a copy of your records in your preferred format (paper or electronic).
- Request corrections to inaccurate or incomplete information in your records.
- Understand how your health information is shared and used.
We comply with all HIPAA regulations to protect your privacy and the confidentiality of your medical records.
Requesting Your Medical Records
You can request your medical records through any of the following methods:
In Person
- Visit Brooklyn Plaza Medical Center during regular business hours.
- Complete a HIPPA Release Authorization Form at the front desk.
- Bring a valid photo ID for verification.
By Email
- Please download, complete and email your completed request form to medicalrecords@brooklynplaza.org, include “Request for Medical Records” as your subject line.
- Indicate in your email how you would like your records to be returned to you.
- Records request to an outside entity should be filled on line #2 of the form
- Include a scanned copy of your photo ID for verification.
By Fax

By Mail
Send your completed request form and a photocopy of your photo ID to:

What to Include in Your Request
When submitting your request, be sure to include the following information:
- Your full name and date of birth.
- The specific records you are requesting (e.g., visit summaries, lab results).
- The date(s) of service related to the records.
- Your contact information (phone number and/or email address).
- Your preferred delivery method (mail, email, or in-person pick-up).
Fees and Processing Time
Fees: There may be a small fee for copying and mailing records, as allowed by law. Fees will be discussed with you before processing your request.
Processing Time: Requests are typically processed within 5-7 business days. For urgent requests, please indicate the urgency when submitting your form, and we will do our best to expedite the process.
Protecting Your Privacy
Your privacy is our priority. We require proof of identity before releasing any medical records to ensure your information is shared securely and only with authorized individuals.
