If your insurance plan requires a copay, it is due at the time of your appointment. It is typically listed on your insurance cared. Our team can also inform you of your co-pay when you check-in.
Unpaid Balances
If you have any outstanding balance from a previous visit, we ask that you address it during check-in. Payment plans may be available if needed.

How Does the Sliding Fee Scale Work?
Patients pay a reduced fee for services based on their ability to pay.
Discounts are applied to services, such as primary care visits, lab tests, and more.
The goal is to ensure no one is denied care due to financial barriers.
What to Bring to Apply:
✓Completed Application
✓Proof of income (recent pay stubs (3), tax return, or unemployment letter. SS Award or Benefit Letter)
✓Proof of residency (utility bill, lease agreement, or official mail)
✓A valid photo ID (Driver’s License or Govt Issued ID)
Our patient services team will work with you to determine your eligibility and explain how the sliding fee scale works for your specific needs.
*Note: For our Patients using our sliding fee scale, your reduced payment is also due at the time of service.
