Social Services

The Social Services Department is an integral part of the medical team here at Brooklyn Plaza Medical Center, Inc. (BPMC). Working closely with the medical and other staff, the Social Services Department contributes to the BPMC’s mission of  “Promoting a Healthier Brooklyn” by safeguarding the community’s health. The primary mission of the Social Work profession is to enhance human well-being and help meet the basic needs of all people and their families with the most complex issues from prenatal to geriatrics.

Social workers are skilled professionals who are able to assist patients dealing with psychological, emotional and social issues.  Social Workers are available to help patients and their families in making the most of the services at BPMC.  We are available for immediate supportive counseling from a bio-psycho-social perspective which includes Alcoholism, Domestic Violence, and Child Abuse and referrals to the appropriate treatment agencies for long term care.  We also have a Violence Prevention Program, as well as a community outreach program to get in touch with those individuals that may not seek medical treatment from a traditional location.  Patients are able to schedule appointments or be seen on a walk-in basis.

The Social Services Department offers comprehensive psycho-social assessment and educational information to patients with Diabetes, Hypertension, and Obesity.  We also provide support groups for patients with Diabetes, Hypertension, and Obesity.  During these sessions we discuss valuable information for managing health disparities and overall healthy living, in addition to creating an environment of peer support for those managing their illnesses.  As always, information and support is also available on an individual basis with one of our social work staff.

We offer group and individual counseling to Prenatal and Post-partum patients including planned parenting services, individual psychosocial assessment, and support and referral services.  We have a Young Male Initiative, to assist young males in guidance and referral services.  For our Geriatric patients, we provide support including a Mini Mental Status Exam and Home Health Aide assessment and referral.  Lastly, we are an official field placement site for graduate social work students from various colleges in the New York area. We provide a didactic and practical learning experience for social work students.

If you or someone you know is in need of our assistance, please do not hesitate to contact our skilled staff.
